LegitScript Certification Cost for Addiction Treatment Providers

What LegitScript addiction treatment certification costs per facility, who is eligible, what the standards cover and how it connects to Google ads approval.

If a treatment center wants to run paid search or social ads for drug and alcohol addiction services, it soon runs into LegitScript. Google, Meta, Microsoft and other platforms rely on LegitScript's Addiction Treatment Certification to decide who may advertise in this category. So "how much does LegitScript certification cost?" is usually the first budget question in a rehab marketing plan, and "can we get certified at all?" is the second.

This guide brings together what LegitScript and Google publish about fees, eligibility, standards and the path from certification to running ads. It is general information, not legal or compliance advice. LegitScript's fees and rules change, so check its current pages before you budget or apply.

Why certification matters for rehab marketing

Google's Ads policy restricts the promotion of recovery-oriented drug and alcohol addiction services. In its 2020 policy update, Google said that to advertise these services "you must be certified as an addiction services provider by the LegitScript Certification program," and that providers who are not eligible for certification "are not allowed to advertise for drug and alcohol addiction services on Google." It also said advertisers "must also be certified with Google," which is a separate step that comes after LegitScript.

LegitScript's certification page names Google, Meta, Microsoft Ads and Nextdoor as platforms where certification is relevant to running addiction treatment ads. Its FAQ adds an important limit: certification makes a provider eligible to advertise, but approval is still subject to each platform's own advertising policies, and certification "does not guarantee advertising approval."

In a notice posted in February 2025, Google said it would allow recovery-oriented addiction services ads in Canada as well, effective March 2025, again tied to LegitScript certification.

What LegitScript certification costs

LegitScript prices addiction treatment certification per unique facility that provides in-scope services, with two separate charges: a one-time application fee and an annual certification fee. According to LegitScript's FAQ and certification page at the time of writing:

Tier Application fee (per facility) Annual certification fee (per facility)
Standard tier, 1 to 9 facilities $1,595 $3,095
Individual practitioner pricing $535 $1,070

A few details from the same pages change what an organization actually pays:

  • Larger organizations pay less per facility. LegitScript's certification page gives application fees ranging from $1,395 to $1,595 per facility and annual fees from $2,550 to $3,095 per facility, depending on volume. LegitScript says organizations with 10 or more facilities may qualify for Enterprise Addiction Treatment Certification.
  • The application fee is nonrefundable. LegitScript's certification page says it is "nonrefundable, regardless of outcome."
  • The annual fee is due only after approval. The FAQ states: "The annual certification fee is due only if your application is approved."
  • Individual practitioner pricing is assigned, not chosen. The FAQ says this tier is set automatically during enrollment. An older LegitScript announcement from June 2020 describes it as being for practices with no more than three practitioners, one website and one physical location, unaffiliated with larger treatment organizations. That older page lists lower prices ($495 and $995), so treat it as historical and rely on the current FAQ for figures.
  • Expedited review costs extra. LegitScript's certification page lists expedited processing for an additional $2,500, with review starting within two business days of submission.
  • Adding a location later is not free. The FAQ says a new location does not need a new application, but it does need a compliance review and payment of the applicable application fee.
  • Fee waivers may exist. Google's policy notes that LegitScript charges for processing and monitoring applicants, "but fee waivers may be available in certain circumstances." Ask LegitScript directly whether your organization qualifies.

A simple way to estimate your own cost

Because the fees are per facility, the basic arithmetic for a small group is:

  • First-year cost = (application fee + annual fee) x number of facilities, plus any expedited fee.
  • Each later year = annual fee x number of facilities, plus application fees for any new locations.

Plug in the tier that applies to you from LegitScript's current FAQ. Budget separately for the internal time it takes to gather licenses, policies and website changes.

Who is eligible, and who is not

The fee question only matters if the organization can be certified. LegitScript's FAQ lists clinical substance use disorder treatment providers as eligible, including outpatient, residential, telehealth and co-occurring treatment, along with in-person credentialed interventionists, mutual support groups and crisis hotlines that do not promote or direct people to specific treatment providers.

The FAQ lists several kinds of organizations as generally not eligible, including:

  • Sober living homes and other non-clinical recovery residences.
  • Lead generation websites.
  • Organizations that redirect prospective patients to treatment providers in exchange for compensation.
  • Peer support and coaching services, DUI education, family-only support groups and assessment-only services.
  • Providers outside the United States and Canada.

For marketers this has a direct consequence: an agency or directory generally cannot get certified to advertise on behalf of "rehab near me" leads it sells to others. The treatment provider is the one that must be certified.

What the certification standards cover

LegitScript evaluates applicants against a published set of standards. Its certification page summarizes them, and they are worth reading before you pay any fee, because a denied application does not get the application fee back. Among the areas covered:

  • Licensure and registration. The applicant must be "adequately licensed for the services they offer and in the jurisdictions they serve," and duly registered as a corporate entity or legal person.
  • Legal compliance. Complying with all applicable laws and regulations, including those on prescribing and dispensing medications and controlled substances.
  • Background and history. Disclosing litigation covering "the past ten years." LegitScript says prior offenses or other bad acts may be disqualifying in its sole discretion.
  • Privacy. U.S. applicants that handle protected health information must comply with all applicable HIPAA requirements.
  • Transparency and advertising honesty. Applicants must ensure advertisements are "accurate, transparent, and not misleading to the public in any way."
  • Incentives and patient brokering. LegitScript says applicants that offer incentives such as paying for travel will generally not be eligible, and lists paying for patients' rent or health insurance premiums as other problematic incentives.
  • Lead generators. Co-ownership or business affiliation with lead generators is prohibited and may be grounds for immediate denial.
  • Staff qualifications and medications. Clinical staff need documented expertise and training in addiction treatment, and only regulator-approved medications may be used or promoted.

Certification is also not a one-time event. The FAQ says certified providers must notify LegitScript within 30 days of significant changes, including business operations, licensure and accreditation, treatment providers, and legal or regulatory matters.

How to get LegitScript certified: a practical sequence

LegitScript runs the application through an online account, and its own pages are the authority on the exact steps. A sensible preparation sequence, based on the standards above, looks like this:

  1. Confirm eligibility first. Compare your services with the eligible and ineligible lists in the FAQ. If you operate a recovery residence alongside a clinical program, check how each part is treated before applying.
  2. Gather licenses and registrations for every facility and every jurisdiction you serve, plus DEA registrations where prescribers handle controlled substances.
  3. Audit your websites. Remove claims you cannot support, make sure facility names, addresses and services match your licenses, and confirm your privacy policy is current. Your HIPAA compliance review belongs in this step.
  4. Review incentives and partnerships. Document any offers to patients and any relationships with marketers, call centers or referral sources. Anything that looks like paying for referrals needs to be resolved before applying.
  5. Prepare disclosures about past legal, regulatory or civil matters for the organization and key staff.
  6. Apply, pay the application fee, and respond to questions from reviewers promptly.
  7. After approval, apply for Google's own certification and set up campaigns within the platform's healthcare policies.

Paid ads are only one channel

Even with certification, ad approval is not guaranteed, and every click is a recurring cost on top of the certification fees. It often makes sense to balance paid search with organic visibility: a fast, clear website, accurate local listings and helpful content. Key takeaways

  • LegitScript certification is the gateway to advertising addiction treatment on Google and other major platforms, and Google also requires its own certification afterwards.
  • Fees are charged per facility: a nonrefundable application fee plus an annual fee that is due only after approval. Check LegitScript's current FAQ for the figures that apply to you.
  • Sober living homes, lead generators and referral-for-pay businesses are generally not eligible.
  • Prepare licenses, website content, privacy policies and incentive disclosures before you pay to apply.
